When you arrive for your first visit be prepared to fill out some paperwork and provide us with your insurance card, prescription for therapy from your doctor, and any other information pertinent to your situation.
After you have completed the necessary paperwork, your therapist will personally escort you to one of the exam rooms for your initial evaluation.
After your assessment, your therapist will provide you with an explanation of your diagnosis, what therapy involves and recommend a treatment plan frequency.
We will then have you set up appointments with our receptionist. Please be sure to bring your calendar with you to the appointment.
Please wear that clothing that provides access to the area being evaluated.
For subsequent appointments, comfortable clothing that will permit stretching and exercise is required.
